Role of Medical Director

  • Co-certifies the terminal prognosis with the attending physician
  • Leads the interdisciplinary team in the development of a plan of care
  • Provides consultation to other physicians regarding Hospice care

Role of Registered Hospice Nurse

  • Assess patient and family needs
  • Develop plan of care to meet identified needs
  • Coordinate team visits & ensure implementation
    of approved plan of care
  • Coordinate care with attending physician, primary
    care giver and/or Care Center Staff
  • Ensure symptom control & pain management
  • Provide patient/family teaching as needed

Role of Attending Physician

  • Certifies the terminal prognosis
  • Assess patient needs, manages symptoms, prescribes treatment
  • Directs and approves the plan of care
  • Coordinates care with interdisciplinary team

Role of Social Worker

  • Assess patient & family emotional, social, spiritual & financial needs
  • Develop a plan of care to meet identified needs
  • Provide direct counseling or refer patients and families to appropriate community agencies
  • Provide Bereavement support

Role of Home Health Aid

  • Provide direct personal care to patient
  • Provide comfort measures, report identified needs to RN Case Manager
  • Provide emotional support to patient and family

Role of Volunteers

  • Provide companionship, support to patient and family
  • Provide needed non-medical services, letter
    writing, errands, respite time for family
  • Provide support at time of death & during bereavement

Role of Chaplain

  • Assess patient & family spiritual needs
  • Develop a plan of care to meet identified needs
  • Provide direct counseling
  • Assist with memorial preparations
  • Provide consultations to community clergy
